Can You Take Fish Oil With Warfarin?

Fish oil (omega-3) and warfarin can both thin the blood. Here is what the evidence says, what to watch for, and how to time them safely.

Yes, most people can take fish oil with warfarin — but only at steady, modest doses, and only with INR monitoring. The risk isn't fish oil itself; it's *changing* your fish oil routine without telling your prescriber. Both thin the blood through different mechanisms, so they stack additively.

How the interaction works (mechanism)

Warfarin works by blocking vitamin K-dependent clotting factors (II, VII, IX, X) in the liver. That's the only thing it does — it doesn't touch platelets at all.

Omega-3 fatty acids (EPA and DHA) work somewhere completely different: they reduce platelet aggregation and shift prostaglandin balance toward less-clotty TXA3 instead of TXA2. Platelets are the cells that physically plug a cut; making them less sticky means a small wound bleeds longer.

So warfarin slows the clotting cascade, and fish oil slows the platelet plug. Neither pathway compensates for the other. Stack them and bleeding time extends more than either alone — even when your INR (which only measures the warfarin pathway) looks fine.

This is why INR alone can be misleading on this combo. A "normal" INR of 2.5 on warfarin + 4 g/day fish oil can bleed like an INR of 3.5 on warfarin alone.

  • Sudden stops: Less discussed but equally important — if your warfarin dose was set while you were taking 3 g/day fish oil, stopping abruptly can drop your INR below the therapeutic range and raise stroke/clot risk. Taper, don't quit cold.

Read the label carefully. A "1000 mg fish oil" capsule usually contains 300 mg EPA+DHA, not 1000 mg. The rest is other fats. Three capsules of that product = ~900 mg EPA+DHA, well under 1 g.

Safer alternatives if you're worried

  • Algae-based DHA: Same omega-3 backbone, similar bleeding profile — not safer just because it's plant-based.
  • Krill oil: Same EPA/DHA mechanism. No proven advantage on warfarin.
  • Dietary fish: Eating salmon/sardines 2-3x/week delivers ~500 mg/day EPA+DHA — well within the safe tier and the cardiology-preferred route for warfarin patients.

If your prescriber wants you on omega-3 for triglycerides or post-MI cardioprotection, they may keep you on prescription-grade icosapent ethyl (Vascepa) with closer INR monitoring — that's a clinical decision, not a self-supplementation one.

Warning signs to act on

Bleeding from this combo usually shows up gradually. Watch for:

  • Bruises that appear without remembered trauma, especially on the trunk
  • Nosebleeds lasting >10 minutes
  • Pink or red urine, black/tarry stools
