Ginkgo Biloba and Aspirin: Bleeding Risk, Documented Cases, and Safer Use

The mechanism (why this isn't theoretical)

Aspirin irreversibly acetylates COX-1 in platelets, blocking thromboxane A2 production for the platelet's entire ~10-day lifespan. That is its anti-clotting effect.

Ginkgo's standardized extract (EGb 761, the form used in most studies) contains ginkgolide B, a potent platelet activating factor (PAF) antagonist. PAF is a separate platelet aggregation trigger that aspirin does not block.

When both are taken together, you have:

  • COX-1 pathway blocked (aspirin)
  • PAF pathway blocked (ginkgo)
  • Net effect: platelets have fewer working activation routes → longer bleeding time → higher risk of spontaneous or trauma-related bleeding

  • Postoperative bleeding in a patient taking ginkgo who underwent laparoscopic cholecystectomy (Fessenden et al., *American Surgeon* 2001).

Most cases involved ginkgo alone or ginkgo + aspirin, with normal platelet counts and normal coagulation labs before the event. Standard tests (PT/INR, aPTT) do not reliably catch ginkgo's effect — bleeding time and PFA-100 are more sensitive but rarely ordered.

  1. Age — vascular fragility and reduced renal/hepatic clearance.
  2. Concurrent medications — NSAIDs, SSRIs, fish oil all add platelet effects.
  3. Product variability — non-standardized ginkgo extracts can contain very different ginkgolide concentrations.
  4. Duration — most trials are short; case reports follow weeks to months of use.

How long should I stop ginkgo before surgery or dental work?

Are some ginkgo products lower risk than others?
Standardized extracts (labeled EGb 761 or '24% flavone glycosides, 6% terpene lactones') have at least predictable ginkgolide content. Non-standardized 'ginkgo leaf' capsules vary widely — some contain almost no ginkgolide B, others contain much more than the label states. Lower variability does not mean lower risk; it means the risk is more predictable. If you must take ginkgo on aspirin, use a standardized product and don't switch brands frequently.
What about combining ginkgo with newer blood thinners (Eliquis, Xarelto, Pradaxa)?
Higher concern than aspirin, not lower. DOACs already carry meaningful bleeding risk, and case reports of bleeding with ginkgo + DOAC combinations exist. Unlike vitamin K vs warfarin, there is no antidote-style monitoring lab for most DOACs, so a bleed can progress before it's caught.
