# How to Check Whether Your Supplements Conflict With Your Prescription Drugs
> Quick answer. Most supplement–prescription conflicts fall into four patterns: absorption chelation (calcium/iron/magnesium binding a drug in the gut), CYP enzyme interference (grapefruit, St John's wort), additive bleeding risk (fish oil, ginkgo, turmeric, vitamin E, CoQ10 with warfarin/DOACs/aspirin), and additive pharmacology (magnesium with blood-pressure drugs, potassium with ACE inhibitors). A 5-minute self-check catches the majority of them before they hurt you.
You do not need to memorise a drug-interaction textbook. You need a repeatable checklist you actually run every time your supplement stack or your prescription changes.
The 5-minute self-check
Step 1. Write down everything you take, with doses
Include: prescription drugs, over-the-counter drugs (including antacids and PPIs), all supplements (vitamins, minerals, herbs, protein, pre-workout, sleep aids), and regular daily coffee. Note the elemental dose where possible (e.g. "calcium citrate 1,250 mg = 250 mg elemental calcium") and the time of day you take each item.
Most conflicts are invisible until you see the full list on one page. A patient may not remember they take TUMS at bedtime, or that their multivitamin contains 18 mg iron, until they physically write it down.
Step 2. Flag the four high-risk categories
Circle anything on your list that falls into these buckets:
- Minerals in supplement doses: calcium ≥300 mg, iron ≥18 mg, magnesium ≥200 mg, zinc ≥25 mg.
- Bleeding-risk supplements: fish oil ≥2 g EPA+DHA, ginkgo, garlic (concentrated extract), turmeric/curcumin, vitamin E ≥400 IU, CoQ10, ginger extract, nattokinase.
- CYP-active herbs: St John's wort, grapefruit (juice or whole fruit daily), goldenseal, Ginkgo biloba (also CYP), high-dose green tea extract, milk thistle.
- Additive-effect supplements: magnesium with blood-pressure drugs; potassium with ACE inhibitors/ARBs/spironolactone; melatonin with sedatives; L-tryptophan/5-HTP with SSRIs.
If nothing on your list falls into these, the conflict risk is genuinely low. If anything does, continue.
Step 3. Check the top pairings for your prescriptions
For each prescription drug, look up whether it appears in any of the following short lists — these are the pairings that generate the majority of real clinical harm:
- Anticoagulants (warfarin, apixaban, rivaroxaban, dabigatran, edoxaban) + aspirin/clopidogrel: avoid fish oil ≥2 g, ginkgo, turmeric/curcumin, vitamin E ≥400 IU, garlic extract, CoQ10 (weak but documented), St John's wort.
- Levothyroxine (Synthroid/Euthyrox): separate by ≥4 hours from calcium, iron, magnesium, aluminium antacids, PPIs, soy protein, high-fibre supplements.
- Statins (atorvastatin, simvastatin, lovastatin): avoid grapefruit ≥250 ml/day; monitor red yeast rice (same class); St John's wort lowers plasma levels.
- Antibiotics (ciprofloxacin, doxycycline, tetracycline, minocycline): separate by ≥2 hours from calcium, iron, magnesium, zinc, dairy.
- Blood-pressure drugs (any): magnesium and potassium additive; liquorice raises BP; ephedra/ma huang contraindicated.
- SSRIs/SNRIs: avoid St John's wort (serotonin syndrome), high-dose 5-HTP, L-tryptophan.
- Immunosuppressants (tacrolimus, cyclosporine): avoid St John's wort, grapefruit, echinacea.
- Levodopa (Parkinson's): high-dose vitamin B6 (≥10 mg) reduces effect if taken without carbidopa.
Step 4. Check dosing timing, not just presence
Many "interactions" are actually just co-ingestion problems that disappear with a 2–4 hour gap. Before you drop a supplement, ask: can I keep it and just move it?
- Calcium/iron/zinc/magnesium vs levothyroxine: separate by 4 hours.
- Calcium/iron vs quinolone or tetracycline antibiotics: separate by 2–4 hours.
- Fibre/psyllium vs any oral drug: separate by 2 hours.
- Coffee vs levothyroxine: separate by 60 minutes.
Time-separation is the fix for absorption interactions. It does not help with CYP enzyme interactions (grapefruit, St John's wort persist for hours to days) or additive-effect interactions (bleeding, hypotension, serotonin).
Step 5. Ask the pharmacist, not the internet forum
Your community pharmacist is the single most under-used resource in outpatient care. They have interaction-check software, they see your full dispensing history, and the check is free. Bring the written list from Step 1. Ask specifically: "Any of these supplements interact with my prescriptions?" and "Any timing changes I should make?"
Do not rely on random forum answers, TikTok, or "I asked ChatGPT". Use the pharmacist as ground truth and use apps and articles as pre-work.
Red flags that need same-week medical review
Stop and contact your prescribing clinician if, after starting or changing a supplement, you notice:
- New bruising, nose bleeds, or blood in stool/urine (bleeding-risk supplements).
- Return of hypothyroid symptoms — fatigue, weight gain, cold intolerance (levothyroxine absorption).
- Muscle pain, dark urine, unusual weakness (statin + grapefruit or red yeast rice).
- Confusion, tremor, sweating, agitation (SSRI + St John's wort/5-HTP — possible serotonin syndrome, seek urgent care).
- Sudden BP drop, dizziness on standing (magnesium/potassium + BP drugs).
- Loss of transplant graft function or organ-rejection symptoms (immunosuppressants + St John's wort/grapefruit — urgent).
These are not "wait and see" symptoms. They are prompts to stop the supplement, keep the prescription, and call the clinician.
